Special Populations Network

The Special Populations Network (SPN) at 海角社区 provides essential services for adults with intellectual disabilities, also referred to as APD consumers through the Agency for Persons with Disabilities (APD). This includes adults with autism, cerebral palsy, Down syndrome, Prader-Willi syndrome, Phelan-McDermid syndrome, spina bifida聽and other conditions as determined by APD.

All of our programs are designed to support these individuals in leading fulfilling, independent lives with the assistance they need.

Residential - Adult Foster Care

海角社区’s CIRCLE Program provides Residential Habilitation (Res Hab) services, also known as Therapeutic Foster Homes or Adult Foster Care.

Adult foster care is a form of long-term care where adults reside in private homes and are assisted and trained to be as independent as possible. 聽Our foster homes for adults provide a family-like environment while including those served in household events and helping them participate in community events.

This program offers supportive, structured living environments for adults with intellectual disabilities who require assistance with daily living skills. Placement in this program must be approved through APD.

Supported Living

海角社区’s Supported Living program helps APD consumers live independently in their own apartments or homes, either alone or with a Supported Living Coach.聽 Services include:

Grocery shopping assistance
Medication management
Bill payment support
Daily living skills training
Arranging for a roommate or paid support if needed
Doctor appointments

Qualification is through APD.聽While this Supported Living program serves individuals with intellectual disabilities, 海角社区 also works with HUD Housing First to help individuals who are unhoused with mental health and/or substance use needs.

Supported Employment

海角社区’s Supported Employment program helps APD consumers find meaningful jobs and ongoing support to ensure job retention.

We assist with job searches, application processes and workplace accommodations, helping individuals gain and maintain employment successfully.

As part of – a – our team members also include , which provides jobs and training for people with significant disabilities.

Court Liaison Services

海角社区 offers Court Liaison Services to support APD consumers who have been arrested and charged with a felony. Our team guides individuals through the legal system, providing competency training and advocacy.

Unlike forensic training, which serves mental health populations, this service of 海角社区’s Special Populations Network specifically supports individuals with intellectual disabilities. Please note, we do not provide services for misdemeanor charges.
